There are 11 Fox Dens, marked on the map once you are near. At each, follow the fox to its Inari shrine and pray there (swipe down on Touchpad).
The trophy comes with the mask, not the last den. After all 11, go to the Hunting Camp Hideout in the north-east of Teshio Ridge, reached through the Sarufutsu Lighthouse. At its locked door, turn the two fox statues on the left to face each other, and the two on the right to face each other. Place the small fox figures that this frees on the pedestals at the door. Follow the path behind it and the bear cub at its end. The woman who comes out gives you the mask.
