Play the second run on Casual with Infinite Ammo (Guns) on, together with Minimalist. Choose Release Elpis at the end: Destroy Elpis does not count.

The timer stops in the pause menu. Check a route or a puzzle there, never while the game runs.
After the first ending, the Challenges list in the Bonuses menu shows the time so far.
Run past enemies instead of fighting them. Health recovers on Casual.
A three-hour pace reaches Grace's Basement at about 0:53, Leon's arrival in Raccoon City at 1:25, the end of the motorcycle chase at 2:05 and ARK at 2:27.

The run also unlocks the Infinite Ribbon for the Insanity run.
