The Light Concentrate is the sphere's beam, which Sophia gets during Chapter X, Parallel Paths (sub-chapter A light that burns). The creature is the giant worm-like monster that hunts Sophia from Chapter VI. Hits on it with the beam count, ten times in all.

Go to Chapter XIII, As One, sub-chapter The Labyrinth, where the creature chases Sophia through a maze.

Stand at one of the lights.
Hold R1 to aim the sphere and press until it fires the beam.
When the creature comes, hit its glowing blue head. It pulls back.
Draw it out again and repeat until the trophy unlocks.
