Five attacks in one turn is easiest with a Fighter such as the companion Lae'zel:

From level 5, a Fighter's attack action makes two attacks.
The Fighter's Action Surge gives a second action in the same turn, for four attacks.
Haste, cast by another character or from a Potion of Speed, gives a third action, for six.

From level 11 a Fighter makes three attacks per action, so Action Surge alone is enough.
