Only the Dark Urge, an origin you choose at character creation, can earn this. In this route it is part of the second run.

In Act 3 the Dark Urge has to defeat Orin in a duel in the Temple of Bhaal, under the Lower City. When Bhaal speaks to you afterwards, accept his gift. You become his Slayer and the trophy unlocks. Accepting is also what opens
Sins of the Father.
Killing Isobel at the Last Light Inn in Act 2 earns it earlier, but you still need to accept Bhaal after Orin for the ending.
Watch out
Do not let your character go down in the fight with Orin, or the Bhaal ending closes.
