Scratch is a dog guarding his dead owner's body in the woods north-east of the Blighted Village in Act 1, near the owlbear's cave.

Calm him with a Perception, Intimidation or Persuasion check, or speak to him after drinking a Potion of Animal Speaking. Do not shout at the body.
Let him have your scent, and he moves to your camp after your next rest.
Pet him whenever you are at camp. After a while he brings a ball.
Take the ball from him with a Strength, Sleight of Hand or Animal Handling check.
Throw the ball somewhere open in camp with the Throw action. The trophy unlocks when he brings it back.
