Karlach is a tiefling barbarian on the Risen Road, north-east of the Blighted Village in Act 1. The date comes in Act 3 if her romance has gone well. Do not play as Karlach, and romance nobody else before the date.

Missable: Karlach's romance runs from Act 1, and the date is early in Act 3. Never join the raid on the grove.
Hidden: PlayStation hides it until it is earned.

Recruit Karlach, then kill the paladins hunting her, led by Anders in the toll house on the Risen Road. If they are alive when you go past the Mountain Pass or into the Shadow-Cursed Lands, she leaves.
Give Dammon, the tiefling smith in the Emerald Grove, a piece of infernal iron to work on the engine in her chest. One is in the apothecary basement in the Blighted Village, another in the treasure room behind Dror Ragzlin, a goblin leader.
Keep her approval high: she likes helping others and dislikes needless cruelty. Never join the goblins' raid on the grove, or she leaves.
At the party after the grove is saved, talk to Karlach before anyone else and spend the night with her.
In Act 2, give Dammon a second piece of infernal iron at the Last Light Inn. Afterwards she can touch people, and another night with her follows.
In Act 3, take a long rest after entering the Lower City. Karlach asks you on a date. Accept and choose the interested answers.
