Kill the Surgeon before he performs surgery on you in combat.

The Surgeon, Malus Thorm, works in the House of Healing in Reithwin Town in Act 2, with four nurses. He operates by calling for a tool from a nurse, so the fight is about keeping the nurses dead. Talking him into his own death does not count: you have to kill him in combat.

Talk to him and pass two Persuasion checks: say the nurses are not ready and will make you sick, then suggest they practise on each other. The nurses kill each other.
Attack him.
Each turn he revives one nurse instead of operating. Kill her again before his next turn, and spend every other attack on him.
