Health upgrades come in pieces. Every three pieces raise your health once, so four upgrades take twelve pieces, and there are far more than twelve.

Each of the six Secret Hideouts (Backroom Espionage) has one in its basement.
The rest are in large resource crates with a clown design, marked on the map once you are close. One stands near each zone's vendor.

Open every large crate you pass in the story, and this usually comes before the side content is done.
