The Three Fingers, who grant the Frenzied Flame, sit behind a door deep under Leyndell, Royal Capital.

In Leyndell's sewers, the Subterranean Shunning-Grounds, defeat Mohg, the Omen in the Cathedral of the Forsaken. His guide says how to get there.
Hit the altar at the back of the cathedral. It slides away and opens a shaft.
Drop down the coffins sticking out of the shaft's walls to the bottom and touch the Frenzied Flame Proscription site of grace. Leave the door beside it shut.
After the final boss, back up the save and see the other two endings first (Step 9 of the Guide and Roadmap).
On the last copy of the backup, fast travel to Frenzied Flame Proscription. Take off all your armour (Options, Equipment), open the door and accept the flame from the Three Fingers.
Fast travel to the Fractured Marika site of grace, touch the statue and choose to become the Lord of Frenzied Flame.

Watch out

Once you hold the flame, no other ending is offered in that playthrough.
