Relics and sword hilts are the same thing: 14 special attacks, used with L1 and and listed under Weapons > Relic. Three come with the story. Huldra Project #9 costs 10,000 Hacksilver at a shop (Special Items > Relics). Four more lie in the realms or drop from optional enemies, and the checklist has each place. The other six take more:

Four are crafted at a shop. Each needs a material that a particular enemy drops, plus all four Lost Pages, lore scrolls counted under Goals > Labors > The Lost Pages.
The Hilt of Skofnung comes from King Hrólf (Grave Mistake).
The Hilt of Forsbrandr comes from the Raven Keeper at Niflheim's Raven Tree, who appears once you have destroyed all 48 of Odin's Ravens, perched or flying around the realms: hit each with a thrown axe.

The checklist holds the relics, the four Lost Pages and the 48 ravens. Some places need your companion's sonic arrows (aim with L2, shoot with , switch type with ), which break glossy green or blue blockages, or a fire bomb, picked up from a cauldron and thrown at gold rubble.

Watch out

Mótsognir's Call, on the top floor of Niflheim's Aesir Prison Wreckage, is not counted in the realm overview. A finished realm does not mean you have it.
A material you did not pick up after a fight waits in the blue chest at any shop.

PowerPyx's relic guide has screenshots of each relic.
