Parasites are items called Mind Flayer Parasite Specimen. They are common: they sit in jars and on desks, and most people the cult of the Absolute controls carry one, including the three leaders of the goblin camp in Act 1.

You can only consume one after your dream visitor, a figure who speaks to you in your sleep, tells you to use the tadpole's power. That conversation comes during a long rest, usually the first one after you reach the goblin camp. Then consume a parasite from your inventory and unlock any power in the illithid powers menu.
