Shovel is a quasit, a small demon you summon from a scroll found in the Blighted Village in Act 1:

In the apothecary by the Blighted Village waypoint, open the hatch behind the counter and go down into the basement.
Find the lever (behind a stack of boxes by the small stairs). It opens a hidden room of coffins behind a bookcase.
Open only the first coffin on your right, against the rock wall. The others hold skeletons that attack.
Take the Scroll of Summon Quasit and use it.

The trophy unlocks when she appears. It is the only such scroll in the game, so do not sell it.
