This romance happens in Act 3, in the Lower City of Baldur's Gate. Go on Karlach's date (Hot Date) first.

The Emperor is the mind flayer who has been your dream visitor.

During a long rest in the Lower City, the Emperor talks to you. Be kind and supportive.
Take the quest to visit the Emperor's old hideout, under the Elfsong Tavern. Go down through the tavern's kitchen or the sewers, fight off the githyanki raiders and look at every keepsake in the deepest room.
At a later long rest the Emperor comes again. Respond warmly and accept every advance.

The trophy unlocks with the scene.
